Post Diagnostic Support Link Worker

A vacancy has arisen for a Post Diagnostic Dementia Link Worker to work in Glasgow

The Link Worker will provide Post Diagnostic Support for people who have received a diagnosis of Dementia and will work with families for a year, providing flexible and proactive advice, information and support around aspects which will enable people to live well with Dementia for as long as possible.

You will provide information and support to people who have received a diagnosis of dementia and their families, signposting and connecting people to peer support or appropriate services for a minimum of one year. This is consistent with Alzheimer Scotland’s 5 Pillars Model of Post Diagnostic Support enabling people to live well with dementia for as long as possible.

The successful applicant will have a recognised professional qualification in social care, social work, nursing, occupational therapy, community work, counselling or other related occupations to a minimum of SVQ Level 4 or Nursing grade 5. Full driving license and access to a vehicle during working time essential.
