Job Description

To provide, alongside other team members, rehabilitative care and support to older adults using the day care services. This post will be peripatetic with work undertaken at any of the locations in response to service demands.

To enable service users to participate in a wide range of therapeutic and social activities

To uphold social work values and to ensure good working practice including understanding and awareness of health and safety

To promote the independence and develop the skills of service users

To work positively with other agencies and services to ensure a quality provision of care

Contribute to the maintenance of service user records and care plans

The Individual

Experience that would give the post holder insight into issues in relation to working with older people in a community setting
SVQ level 2 in health and Social care
Effective literacy and numeric skills for accurate recording.
Willingness to undertake further training as required
Practical Skills,
Observational Skills to enable effective recording of information relating to service users
Flexibility, Empathy and Approachability.
Experience of working in a re-facilitative way.
Experience of community participation
Self-confident and self-motivated
Ability to work on own and as part of a team
You will be expected to travel efficiently and effectively between various work locations within Moray to meet the operational requirements of the Service. Due to the rural nature of Moray this is normally undertaken by use of a car/van.
Knowledge of Social Care Values. (Include) SSSC, national care standards and National Dementia Strategy Standards.
Ability to form positive relationships with service users, colleagues and other professionals.
Sensitivity and understanding of discriminatory and health and well being issues. Flexible approach to work patterns and bases, i.e. ability to perform outreach work. Ability to respond appropriately in difficult or challenging situations.
Ability to work effectively as part of the day service team and in multi-disciplinary settings.
Ability to use own initiative.
The ability to engage service users in activities and participation within the local community.
Ability to communicate in an open and approachable manner.
Willingness to accept direction/delegation
Appreciation, acceptance and commitment to the importance of confidentiality
Ability to carry out duties that involve personal care of service users.
Ability to carry out moving and handling duties in line with health and safety regulation and risk assessment.
Ability to work flexibly to meet the needs and demands of the service
