£25,264 - £28,380
Various Locations in the Perth & Kinross Council Area
Social Prescribers signpost people to community based supports. As well as improving a person’s quality of life, the role will be working with front line statutory workers such as Social Workers, District Nurses, Occupational Therapists, CPNs and GPs to inform them of community based supports in the area, identify people on their caseloads suitable for community based supports and signpost them where appropriate.
The role will help to increase the numbers of people accessing community based supports in Perth and Kinross. This will help improve outcomes for people, especially those who do not meet the eligibility criteria for statutory supports.
This is a demanding post that will require you to demonstrate flexibility and skills to respond to the changing needs of individuals.
Ideally, you will be able to demonstrate experience of multi-disciplinary working and be committed to working closely with colleagues from other disciplines.
You must be able to work effectively within a team as well as individually and demonstrate a commitment to improving service provision.
You will receive referrals from front-line staff and partners and co-ordinate and maintain networks and engagement.
You will work well with service users to promote community based services and other supports as appropriate.
You will be confident and become a main point of contact for social prescribing within the locality.
