£40,243 - £42,946 Fixed Term (for up to 12 months)

You will assist in maintaining vulnerable people in the community by providing a range of social work and care management tasks. These include carrying out Outcome Focused Assessments, the negotiation of flexible packages of care, and the monitoring and reviewing of individual care packages.  You will mainly be responsible for monitoring and reviewing people who reside within their own home in our locality however part of the role may include reviewing people who reside within care homes. You must have experience of assessment of Social and Personal Care needs and be able to demonstrate a sound knowledge and understanding of social work principles and values. 

 

The Individual

The ideal candidate will be motivated, engaging and possess excellent interpersonal and communication skills. Effective time management is essential for a successful autonomous worker to manage a caseload with the ability to be flexible and respond to crisis / competing demands being integral to the role. You must have experience of providing individual needs-led assessment and reviews for service users and their carers within Eligibility Criteria and Operational Guidelines. You must be able to accept referrals identified as appropriate to the skills and expertise of a Social Worker for assessment and review. You must have experience of assessment and review of Social and Personal Care needs and must be able to demonstrate a sound knowledge and understanding of social work principles and values. Experience of Adult Protection Procedures. You must have a degree in Social Work,    
you must have a driving licence and access to a car for work. 
                     
This post may be suitable for candidates who have recently completed their Social Work Degree but have not yet graduated. In this circumstance, a Social Work Assistant contract will be offered until you receive your qualifying certificate and are eligible to register with the SSSC as a Social Worker.
