Where this is a requirement for the successful candidate to work from home as part of the role  you will be provided with the necessary IT equipment by the Council, however, you will be responsible for the provision of any required furniture.

Role Purpose

An opportunity has arisen within the SW START Team for the post of Assistant Team Leader, based in Borders General Hospital.

You will work alongside Social Workers, paraprofessionals and NHS colleagues, undertaking assessment, care management and practical interventions to safely discharge service users from hospital to home. You will provide appropriate advice, guidance and practical support to individual carers and adhere to legislation, standards and procedures and ensure effective use of resources.


Applicants must possess a recognised professional qualification in Social Work, Occupational Therapy, or Nursing

A current driving licence and use of a vehicle is an essential requirement of this post.
