Hands-On Homecare provide a service to older people and adults with physical and sensory impairment living in their own homes and in the community.
We actively involve people in their own support from the start as service users are experts of their own experiences, needs and wishes. A person's personal plan (sometimes referred to as a care plan) should be right for them because it sets out how their needs will be met, as well as their wishes and choices. We have in place detailed care plans for the people we support in their own homes.
