Senior Community Link Worker

An exciting position has become available for a Senior Community Link Worker based in our established service in West Lothian. The successful candidates will earn £15.50/hour.

You will support the service manager in the day to day running of 2 well-being hubs, based in Bathgate and Livingston.

You will be based in one of the Hubs leading a small team of Link Workers and working positively as part of a small MDT including psychology, occupational therapy and mental health nurses..

You will conduct initial assessments of people’s needs and also deliver person centred individual support to people in contact with the hubs, who will be experiencing mental health issues possibly exacerbated by stressful social circumstances that are having a negative impact on their mental health and well-being.

You will support people to help them identify and achieve personal outcomes and priorities that they would like to address and link them to local and national resources and services as required.

You will also deliver and support staff to deliver psycho educational groups.

Applications from people who have personal experience of mental health difficulties are encouraged.

Role Requirements

· Previous experience of working with people in demanding situations would be desirable.

· An understanding of mental health issues.

· Ability to manage people in a non judgemental and supportive manner.

· Excellent communication skills and a positive can do attitude.

· Ability to build rapport easily and maintain positive and effective communications.

· Able to work independently in a proactive manner.

· Able to sustain contact with individuals to encourage and motivate them.

· Confident in working closely with clinical staff, G.P.’s, practice staff and community agencies.

· Able to maintain accurate records.

· Basic IT skills, working knowledge of office 365 desirable.

Role Responsibilities

· · Conducting initial structured conversations with people referred to service using standard internal paperwork and processes including introducing fellow staff to undertake support duties on completion of assessments, as well as liaising with clinical colleagues and other agencies where necessary.

· · To provide individual support to service users in accordance with their agreed and pre-set plans and fulfil key worker duties as required.

· · To participate with service users in negotiating, agreeing, and reviewing their individual plans.

· · To deliver individual and group based psycho educational interventions

· · To take part in organising, supporting, delivering and reviewing groupings of service users where they may occur.

· · To take a lead role in local community activities that promote positive mental health awareness and assist to challenge stigma and discrimination.

· · To keep accurate and up to date written records of service users visits/work.

· · To be proactive in promoting effective communication with relevant statutory and third sector agencies/professionals and their staff to ensure the best possible support arrangements are in place for service users and connections for future support/information and advice are also in place.

· · To be proactive in relationships with NHS mental health nurses and GP practices ensuring positive working relationships are achieved and suitable referrals are received.

· · To be proactive in promoting and maintaining effective internal and external communications.

· · To lead by example and work positively as part of a team, your actions being conducive to promoting effective, open, honest and supportive teamwork.

· · To organise and take a lead role in team meetings.

· · To provide regular supervision to relevant team members as well as ongoing support to assist them in their work and development, including annual appraisal and continued personal development.

· · To maintain positive and current communications with service manager and fellow senior to ensure service runs in a consistent and effective manner.

· · To deputise for service manager or fellow senior as required in their absence

· · To provide planned and unplanned support, advice and direction to staff as required.
