This is crucial role, working in the field with the care team and clients.

This role has been developed for someone who has care experience within a senior carer role, or has significant experience in home care.

PURPOSE AND KEY FUNCTIONS

To provide and apply a regulated Care service in accordance with the legislative requirements, local guidance and in line with Health & Social Care Standards.

To follow service procedures, delivering direct interventions and support, to staff in relation to client’s specific demands and needs, focusing on obtaining positive outcomes and offering of solutions.

To maintain, audit accurate records of service delivery and report all information according to the policies and procedures within the framework of the business.

To support and empower new staff in locality to achieve confidence through development, observation, guidance and by being a proficient and approachable Senior role model.

RESPONSIBILITIES

Deliver service and client care outcomes in an efficient and effective way.

To support inexperienced staff to develop their skills and knowledge through supporting them in their care calls to develop the care teams confidence, skills and approach.

To work with the senior team and carers, to develop person centered care plans, and ensure they are relevant to the current needs of the clients.

Carry out care team observations, spot checks and work alongside new staff so they can learn by example to provide quality care to meet Nightingale's standards.

To meet new clients and collate information as required and tasked by the line manager.

To be involved and contribute to clients reviews.

Act as a point of resource on the field to support clients or staff.

To represent the business in a professional manner within client’s homes

JUDGEMENTS AND DECISIONS

To liaise with the management team in relation to any acute staff issue.

Contribute to the construction, implementation, monitoring, and review of appropriate care plans whilst collaborating with colleagues and other agencies to achieve the best outcomes for the clients.

To participate in staff support, record accurate observations, report actions relevant to training and development requirements of the Care team.

ESSENTIAL

Professional qualification in Health & Social Care, SVQ 2, or above, or working towards SVQ3.

Registration with the appropriate professional body

Computer literate with an ability to record appropriately and produce suitable documents and reports.

Good organisational skills and the ability to prioritise demands

Ability to work with other agencies and disciplines in a partnership and collaborative approach

To provide a valuable senior support to the care team in the field

Ability to apply confidentially and share information appropriately

Good understanding of situations which affect clients using the service.

Ability to behave in a consistent and reliable manner and be a provider of practical support and learning to the Home Care Assistants.

Ability to travel efficiently and effectively where required.

Ability to provide personal care when required.

To manage own workload to maintain momentum and achieve performance management.
