Make a difference where it matters most

An exciting opportunity has arisen to join our Falkirk Tenancy Support Service as a Health & Wellbeing Coordinator.

In this rewarding community-based role, you will support individuals who are experiencing difficulties sustaining their tenancies, helping them to overcome challenges and build the skills, confidence and resilience needed to maintain their home. You will also work alongside individuals transitioning from homelessness into their own tenancy, supporting them to settle successfully and achieve greater independence.

Working in partnership with housing providers, health and social care professionals, and community services, you will deliver person-centred support that focuses on individual strengths, aspirations and outcomes.

What you'll do

  • Provide personalised, outcome-focused support to individuals within their own homes and communities.
  • Support people to establish and sustain successful tenancies.
  • Assist individuals moving from homelessness into settled accommodation.
  • Help people develop budgeting, money management and independent living skills.
  • Work collaboratively with partner agencies to achieve positive outcomes.
  • Complete assessments, support plans and reviews.
  • Promote independence, wellbeing and community inclusion.
  • Maintain accurate records and reports in line with organisational and regulatory requirements.

What we're looking for

We're looking for someone who is passionate about supporting people to achieve positive change and improve their quality of life.

Ideally, you will have experience working in a housing support, homelessness, tenancy sustainment, community support, social care or related environment, supporting individuals to overcome barriers and achieve positive outcomes.

You will be able to demonstrate:

  • Experience of supporting individuals with a range of needs within a community-based setting.

  • An understanding of the challenges faced by people at risk of homelessness or experiencing difficulties sustaining a tenancy.

  • Experience of developing, implementing and reviewing person-centred support plans.

  • The ability to build positive, professional relationships and engage effectively with individuals who may be experiencing complex life circumstances.

  • Experience of working in partnership with statutory and third-sector agencies to achieve positive outcomes.

  • Strong communication, organisational and record-keeping skills.

  • The ability to work independently, manage a caseload and prioritise competing demands.

  • A commitment to promoting independence, choice, dignity and inclusion.
