Lead with purpose. Create better lives.

At Wheatley Care, our purpose is simple: everyone is entitled to live an independent, healthy and happy life, filled with opportunity. We are committed to delivering personalised support that helps people achieve the outcomes that matter most to them while promoting dignity, choice, independence and inclusion.

As we continue our journey as an independent organisation, this is an exciting opportunity to join a values-driven leadership team and play a key role in shaping services that people are proud of.

We are looking for an experienced and compassionate Health and Wellbeing Team Leader who can inspire others, champion innovation, and lead the delivery of outstanding person-centred support.

About the Role

Reporting to the Health and Wellbeing Manager, you will have responsibility for the day-to-day leadership of services, ensuring the delivery of high-quality, personalised support that enables people to achieve their goals and aspirations.

You will work closely with the people we support, families, colleagues and external partners to co-create services that improve health, wellbeing, independence and resilience. You will promote a culture of trust, excellence and innovation while supporting your team to deliver positive outcomes every day.

What You'll Be Doing

  • Leading and motivating a team of Health and Wellbeing Assistants.
  • Promoting a culture of co-creation, participation and person-centred practice.
  • Supporting people to achieve the outcomes that matter most to them.
  • Ensuring outcome plans are meaningful, individualised and regularly reviewed.
  • Coaching, supervising and developing colleagues through outcome-based supervision and workforce development.
  • Building strong partnerships with families, communities and external agencies.
  • Supporting community integration and helping people maximise local opportunities and resources.
  • Driving quality assurance, continuous improvement and service excellence.
  • Championing equality, diversity and inclusion in everything you do.
  • Supporting recruitment, induction, training and performance management activities.

Who We're Looking For

We're interested in hearing from people who:

  • Have experience leading teams within health and social care.
  • Are passionate about delivering outstanding, personalised support.
  • Can motivate and inspire others to achieve their full potential.
  • Demonstrate excellent communication and interpersonal skills.
  • Have experience working with a range of support needs and client groups.
  • Are solution-focused, innovative and committed to continuous improvement.
  • Understand the importance of partnership working and community connections.
  • Can create environments built on trust, inclusion and mutual respect.

Essential Requirements

  • SVQ Level 3 Health and Social Care (or equivalent) or working towards it.
  • Evidence of continued professional development.
  • Professional registration with the SSSC.
  • Ability to work flexibly to meet the needs of the people we support.

Why Wheatley Care?

Our values of Ambition, Trust, Inclusion and Excellence shape everything we do. We listen, learn and work alongside the people we support to deliver care that feels personal and makes a genuine difference.

When you join Wheatley Care, you'll become part of a supportive organisation that:

  • Puts people at the heart of everything it does.
  • Invests in learning, leadership and workforce development.
  • Encourages creativity, innovation and new ideas.
  • Promotes a positive, inclusive culture.
  • Supports colleagues to grow, develop and succeed.
  • Is committed to delivering care that people are proud of.
