You will be required to work 21 hours per week, 52.14 weeks per year.

 

This is a time of significant change for both the Social Work Service in Dumfries & Galloway, and the wider health and social care sector in general. This post offers real opportunities to contribute to service delivery and development.

 

You will be an integral member of the Machars Home Team, where you will work in partnership with colleagues from Adult Social Work, Community Nursing, Occupational Therapy, Physiotherapy, Public Health Improvement and Patient Flow.

Your primary responsibility will be to work with people in the community to assess their need for care and support and to help formulate and review outcome-based plans. This will be carried out in conjunction with Carers and families where appropriate, and alongside colleagues in the Home Team. You will also link closely with colleagues in the wider Health and Social Care Partnership and other agencies. Experience of working in a health or social work setting is essential and experience of assessment and care management is desirable. A good working knowledge of Self-Directed Support would be beneficial.

 

You will need to have excellent communication, assessment, analytical and co-ordination skills. You will also need to be able to work under your own initiative and prioritise your work accordingly. You will have knowledge of the major themes which are relevant to health and social work services, and an awareness of the functional impact of old age, ill health and disability. You will be expected to practice in accordance with legislation, in particular the Social Care (Self Directed Support) (Scotland) Act 2013 and the Adult Support and Protection (Scotland) Act 2007. You will be committed to evidence-based practice and in return, you will receive good support, regular supervision and opportunities for development.

 

A Dip/SW/CQSW/CSS qualification or equivalent is essential as is registration with the Scottish Social Services Council.

 

This post is considered as Regulated Work with Vulnerable Children and Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. From 28 February 2011, preferred candidates will be required to join the PVG Scheme or undergo a PVG scheme update check prior to formal offer of employment being made by Dumfries and Galloway Council.

 

Applicants who have been resident outside the UK within the last 5 years, if offered a position, in addition to a Disclosure Scotland Check, will be required to provide a completed police record check from the police authority in the country or countries in which they were resident.

 

A current driving licence is an essential requirement of this post. However, if you have a disability which precludes you from holding a driving licence, consideration will be given if alternative transport can be arranged.
