It also involves linking with other initiatives including the Integration of Social Care and Health.
  • Providing leadership and vision in the delivery of the objectives of Self-Directed Support and Carer Support.

The work will involve close liaison with a wide range of agencies involved in providing care and support services.

The Post holder will be responsible for guiding the work of multiple simultaneous improvement work streams across a broad range of health and social care topics.

You will work 14.8 hours per week, Thursday and Friday. The post is for second half of the week as other part of post is filled, but there can be flexibility around working hours.
