Job Details
The post is within the Aftercare Team working with care experienced young people age 16-26.
Key Responsibilities include:
Undertaking thorough and informed comprehensive assessments of need and risk.
Working individually and/or collaboratively to produce plans to support young people and their families.
Assessing, managing, monitoring and reducing risks to young people and others.
Contributing to complex assessments of need and risk with an aim of improving outcomes whenever possible.
Carrying a caseload with varying complexity, requiring specialist skills and knowledge to enable appropriate and effective responses to the needs/risks of service users.
Providing written reports as required, including informed recommendations concerning care/action plans.
Attending meetings and case reviews.
Updating and ensuring the accuracy of all relevant young people’s records and taking responsibility for the effective application of information management systems.
Engaging and working in partnership with service users, family members, Police, Health, Housing, statutory/voluntary agencies and others as appropriate.
Leading on group work
Carrying out specialised assessments around risk aimed at reducing risk of harm.
The Person
The successful candidate will be Educated to SCQF level 10, which includes a Degree in Social Work or equivalent.

Before confirming your appointment, you will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland.
As this role requires SSSC registration, the successful applicant will have three months to apply for registration.
If appointed, you must start your application as soon as possible so you're registered within the required six months.
If you need to achieve a qualification to maintain your SSSC membership, information will be provided if you are appointed. You must ensure you complete the qualification within the specified timeframe.
