Highland Health and Social Care Services help people from all walks of life, as well as their families or carers. Health and Social Care in Highland provides help and assistance in many situations arising due to illness, disability, old age or poverty.
An exciting vacancy has arisen within our department to manage the Care at Home team which functions as part of the District Health and Social Care Integrated Team. The Care at Home service enables people to realise their potential through the ethos of Reablement therefore maximising independence and supporting the individual to remain in their own home. The post-holder will be required to take the autonomy for their actions and work unsupervised frequently without immediate access to advice and support from the Care at Home Manager or the Integrated Team Lead.
It is the post-holders’ responsibility to meet the set objectives targets and person-centred outcome measures for the care at home service and individual practitioners.
To create care plans from multi-disciplinary assessments and coordinate and develop care packages for service users with complex needs identifying clear outcomes, monitoring and reviewing care packages every 6 months or as required.
To work within Self Directed Support (SDS) Legislation and deliver a person-centred Care at Home service to meet the assessed need and personal outcomes of those referred to the service in line with the standards required by the Scottish Social Services Council (SSSC) of a registered Care at Home service.
The post-holder will commission individual care packages from local independent providers and provide regular reviews of those packages.
The post-holder will be responsible for ensuring the service meets the requirements of the Scottish Social Services Council legal requirements and the regulatory standards of the Care Inspectorate.
A driving licence is essential for this post and applicants should have access to their own vehicle for work purposes.
This is a permanent full-time post of 37 hours working as per service requirements.
