Job Description
To assist the Community Care team carry out procedures in relation to a client's community care needs assessment, ensuring current policies and initiatives set by Local and Central Government are applied.
Undertake home visits to contribute in the completion of needs assessments in relation to services being provided by Education and Social Care Department.
Requirements
To assist with the completion of community care assessments and other related documents
Liaise with and advise Community Care Staff in regard to visiting clients and verifying information, arranging home visits as required
Liaise, consult & verify with Community Care Staff in regard to the accurate recording of care services entered on Carefirst IT system
Use of various IT systems
Participate in team meetings, attend supervision sessions and participate in ERDP
Dealing with Challenging Relationships and Environment

Responsibilities
The Individual
Experience/Training in Local Government
Knowledge of Local Authority policies in relation to Community Care
Educated to HNC level with English
Knowledge and experience of Microsoft office packages, including Excel and Access
Excellent verbal and written communication skills
To be able to work under pressure & prioritise own workload
Be able to exercise own initiative and make decisions
Effective organisation and time management skills
Self-confident and self-motivated
You will be expected to travel efficiently and effectively between various work locations within Moray to meet the operational requirements of the Service. Due to the rural nature of Moray this is normally undertaken by use of a car/van
Ability to deal confidently and sympathetically with people who may not agree with decisions and policies
Ability to work as part of a team and contribute to service development
Appreciation, acceptance of and a commitment to the importance of confidentiality
Willingness to accept direction/delegation
