Access Team, Pullar House, Perth
Perth & Kinross Council aims to work with the residents of the Perth & Kinross area to provide independent living across all communities.
Access Team within the Early Intervention and Prevention Service has a Social Work vacancy, the team has recently been reviewed with the scope of work focusing on critical and short term interventions along with some signposting. This is an excellent opportunity to join a highly innovative team, the work is extremely fast paced and varies each day with excellent training, supervision provided, average case time intervention being two weeks.
The successful applicant will assist in supporting people within the community by providing a range of social work and care management tasks including carrying out Outcome Focussed Assessments, negotiating flexible packages of care, promoting Self Directed Support, attending meetings and working closely with our colleagues in Housing, Locality Teams, Care providers, Commissioning and Health.
Responsibilities also include statutory work under the Adult Support and Protection Act and delivering a duty service completing direct access to care applications and welfare checks.

Requirements
The ideal candidate will be motivated and engaging and possess excellent interpersonal and communication skills.
Effective time management is essential for a successful autonomous worker to manage a complex caseload with the ability to be flexible and respond to crisis / competing demands being integral to the role.
A good understanding of Self Directed Support and how this is delivered is essential. The ability to integrate and cultivate successful links within the team and across services is highly desirable.
Knowledge of services, challenges, demands, and demographics within PKC would be an advantage.
You must have experience of assessment of Social and Personal Care needs; You must be able to demonstrate a sound knowledge and understanding of social work principles and values; Experience of Adult Support and Protection Procedures and all relevant legislation.
You must have a DIPSW, CQSW, CSS and relevant Social Work Qualifications. You must have successfully completed “Council Officer” in relation to Adult Support & Protection training.
You will have a current registration with SSSC and be eligible to register with PVG scheme.
You must have a valid UK driving licence as this post requires you to travel to a variety of locations across Perth and Kinross.
