SCOPE Team, Pullar House, Perth

An opportunity for the post of Senior Practitioner has arisen for the SCOPE team which is a specialist team working with individuals aged 14+ going through school transitions; whilst also supporting individuals across their whole life span. The individuals we support have a diagnosed learning disability and/or autism and are identified as having complex care needs.

The SCOPE Team is a Health and Social Care multi-disciplinary team. SCOPE stands for:

S – Supporting young people and adults with complex needs
C – Community based approach/assessment
O – Offering young people and adults’ choice in their care packages
P – Person-centred planning
E – Enriching people’s lives

The principal role of the SCOPE team is to provide life-long support for complex cases of individuals who reside across Perth and Kinross. The team is made up from disciplines from both Health and Social Care, such as social workers, Outreach Workers, Psychology and aligned to NHS services that include Speech and Language Therapist, Physiotherapy, Dietetics, Psychiatry and Community Learning Disability Nurses. The team will link with local partners such as Education and Children’s services, Housing, other Health Services and third sector partners.

SCOPE will be leading in developing locally based provision, which will improve the lives of those with complex care requirements as well as their families and carers. The team will have a whole person approach and will embed the principles of positive behavioural support into the delivery of provision.

As a senior practitioner, you will carry a small caseload of complex cases where you will undertake the social work role.

You will support the Team Leader in the discharge of their duties through supervision of staff, screening and allocation of referrals and representation on inter-agency forums.

As a qualified Social Worker with at least two years post qualifying experience, you will work at an advanced level, carrying additional duties associated within identified roles or undertake specified tasks to support the Team Leaders and deputise as required. This role will manage and develop specific areas within the Adult team, and the delivery of best and improving practice, including statutory duties under the Adult Support and Protection Act and Adults with Incapacity Act.

You will provide, manage and/or co-ordinate and contribute to the development of best practice and service development. This will include delivering person centred inclusive services in line with the Health and Social Care Standards and SSSC Codes of Practice, with a focus on Empowerment and Wellbeing, working in partnership with individuals, families, carers, and communities to deliver outcomes and maximise independence.

You must be able to demonstrate initiative and independence, work within defined policy guidelines and be able to deal personally with predictable, and unforeseen, problems. Extensive practical experience with knowledge across a number of areas is required.

You must have a valid UK driving licence as this post requires you to travel to a variety of locations across Perth and Kinross.

The Individual

The successful candidate should hold a degree in Social Work or equivalent, and be a professionally registered practitioner as per SSSC requirements.

Ideally you will have a post qualifying qualification, or willingness to work towards this, and experience in supervision and management, or a willingness to develop leadership and management skills.

You will have experience in public protection work within a legal framework.

You will demonstrate a commitment to ongoing professional development, to acquire the skills and knowledge relating to the role and developing expertise to provide support to others. You should be committed to learning, improvement and innovation and demonstrate a flexible approach to change and an ability to challenge the status quo. A commitment to creating and supporting a safe working environment is essential.
