As a Social Worker / Care Manager in our multidisciplinary Adult Social Work and Practice Team, you will work alongside professionals including Nurses, Occupational Therapists/Assistants, Paraprofessionals, Community Care Assessors and Council Officers, all supported by a strong administrative team. Your role will involve case management; assessing, planning and reviewing our citizens while contributing to Adult Support and Protection investigations. Degree in relevant subject
A current driving licence and use of a vehicle is an essential requirement of this post.
