Job Description

This is a permanent post of 35 hours per week, working 5 days over 7 including evening and weekend working.

We are looking for an enthusiastic individual to work within our day services, alongside our established team of workers, to support older adults to access activities in the community and within our Day Service Buildings.

You will provide a high standard of care and support to individuals and groups consistent with their needs and wishes. You will be responsible for carrying out the day to day activities of the day centre, delivering consistent high standards within the principles of person centred care. This will include integrated partnership working with NHS and Local Authority colleagues, the private and voluntary sector to provide a holistic approach to supports for older adults. You will liaise with the individual, their family, friends and community healthcare specialists to deliver day care and related services to older people and those living with dementia.

You will be responsible for compiling individualised care and support plans and conducting regular reviews of the care/support and progressive development of the care plans to ensure that the individual, changing needs of each person are fully met.

You should also have knowledge of legislation set out by the Scottish Government and National Care Standards.


Requirements

This post is considered as Regulated Work with Adults under the Protection of Vulnerable Groups (Scotland) Act 2007 and as such membership with the PVG scheme is essential. Further details of this scheme can be found here.

Preferred applicants will be required to become members of the relevant PVG scheme, or undergo a PVG Scheme update check prior to a formal offer being made by West Dunbartonshire Council.
