Alltogether Care Services provide health and social care services to adults from the ages of 16+. We are committed to providing the best standard of care and have been awarded excellent grades from the Care Inspectorate. Our Service Users are predominantly frail elderly, people with dementia or age-related illnesses. We also support adults with life limiting conditions, physical or learning disabilities. We support our Service Users by following their Personal Support Plans, assisting them with washing, dressing, toileting, oral hygiene, continence care, medication support, domestic support and much more. A part of our support is working with people individually to assist them to achieve their goals and outcomes.

As a Care Coordinator, you will play a vital role in managing and coordinating the scheduling of care services for service users in need of assistance. You will be responsible for ensuring that service users receive timely and appropriate care by coordinating with Care Workers, the service users, and their families. This role requires strong organisational skills, excellent communication abilities, and empathy towards our service users' needs.

This vacancy is Full Time based on a 5 over 7 working patterns, so you always know your working pattern. The role requires flexibility to work in all our areas/runs, acting as a first point of call for staff and Service Users. You will be predominantly field based for most of your working time with the rest of your time dedicated to administration.

Responsibilities

1. Schedule Care Services: Coordinate the scheduling of Care Workers to provide services to service users based on their individual needs, preferences and care plans.

2. Service User Assessment: Conduct initial assessments of service users to determine their care needs and preferences, and update assessments as necessary.

3. Communication: Maintain regular communication with service users, care workers, and their families to ensure that scheduling arrangements meets the needs of all partied involved.

4. Problem Resolution: Address scheduling conflicts, last minute changes and other issues that may arise to ensure continuity of care for service users.

5. Documentation: Maintain accurate records of service schedules, care plans and communication with service users and care workers.

6. Staff Coordination: Work closely with the recruitment officer to ensure that an adequate number of care workers are available to meet the needs of the service users.

7. Training and Support: Provide training and support to care workers on scheduling procedures, service users preferences, and any changes in care plans.

8. Compliance: Ensure compliance with all relevant regulations, policies and procedures related to scheduling and service user care.

9. Quality Assurance: Monitor the quality of care provided to service users and address any concerns or issues that may arise.

10. Continuous Improvement: Identify opportunities for process improvement in scheduling and service user care coordination to enhance efficiency and effectiveness.

11. Participate in on-call on a roster basis

Person Specification

  • Experience using rostering systems
  • Understanding of the scheduling process
  • Ability to match Service User requirements with the appropriate Home Support Worker
  • Office administration experience - typing, letter writing and filing
  • Working knowledge of quality control procedures
  • Good people skills with the ability to provide an exceptional level of service
  • Ability to negotiate effectively
  • Ability to work both independently and as part of a team
  • Excellent standard of IT skills
  • Proven staff management
  • Driving licence
