About the service

We’re looking for Relief Support Practitioners to join our teams in Auchtermuchty and Kelty to support people across our Housing Support services.

Our Housing Support Services providing support 24 hours a day all year round. Shift patterns vary and include early and late shifts, evenings, weekends and public holidays, there is something to suit everyone.

Responsibilities of the Support Practitioner will include:

  • Healthcare Support: Assist with healthcare needs, including medication administration and personal care, ensuring the well-being of individuals.
  • Record Keeping: Maintain accurate and up-to-date records of daily tasks and activities, ensuring clear documentation of progress and support provided.
  • Collaborative Care: Work closely with colleagues, families, and individuals to deliver high-quality care and support, fostering a team-oriented approach.
  • Encouraging Independence: Promote independence and personal growth by following Support Plans tailored to each individual's needs and goals. This could include supporting them with everyday tasks or activities they enjoy, such as going shopping, attending community groups, enjoying walks, or taking part in hobbies and outings that matter to them.
