The WAYfinder Service provides early intervention and person‑centred support to individuals who may be at risk of crisis or in need of guidance to access the right services. The focus is on prevention, wellbeing, and helping people navigate a pathway towards more stable and positive outcomes.
Our approach is supportive, flexible, and tailored to each individual. We work alongside people to understand their circumstances, identify what matters most to them, and connect them with the support, services, and resources that will help improve their overall wellbeing.
As a Health and Wellbeing Assistant, you will play a key role in providing practical and emotional support, helping individuals to build confidence, develop coping strategies, and access the right pathways. You will work with people who may be experiencing a range of challenges, including social isolation, mental health pressures, or housing‑related difficulties.
The service is built on developing trusting relationships and offering reassurance, guidance, and encouragement. The aim is to empower people to make informed choices, strengthen independence, and prevent issues from escalating wherever possible.
Key areas of support include:
- Providing early intervention and preventative support
- Supporting individuals to access appropriate services and resources
- Promoting wellbeing, confidence, and resilience
- Encouraging independence and informed decision‑making
- Building positive, trusting relationships
- Supporting individuals to engage with their communities and support networks

Who are we looking for?
We are looking for someone who is caring, reliable, and adaptable, with a genuine commitment to supporting individuals in a preventative and strengths‑based way. You will be confident engaging with a wide range of people, able to listen, build rapport, and respond sensitively to individual needs.
Previous experience in a care, support, or customer‑focused role is desirable but not essential, as full training will be provided. An understanding of early intervention, wellbeing approaches, and working in a person‑centred way would be beneficial.
