WAYfinder is seeking a motivated and compassionate Health and wellbeing Co-Ordinator to join our team. This role is central to delivering our core aim: supporting individuals, who are at risk of homelessness, currently experiencing homelessness, or who have previously experienced homelessness, to sustain a tenancy and improve their wellbeing.
You will work closely with individuals to build trust, promote independence, and support long-term tenancy sustainment through flexible, open-ended support.
Who are we looking for?
The ideal candidate will:
- Deliver high-quality, person-led support to individuals with complex needs in line with Housing First principles
- Work collaboratively with individuals, their support networks, and the GCHSCP Housing First Assessment Team to determine access and ongoing support needs
- Provide flexible, open-ended support, recognising that each person’s journey and definition of success is unique
- Support individuals to sustain their tenancy, with the ability to adjust the intensity of support based on their needs and preferences
- Build strong partnerships with statutory services and local community organisations
- Promote choice and independence, including supporting individuals to step down or re-engage with services as required
- Undertake holistic needs assessments and co- produce support plans in collaboration with People We Work For
- Support individuals to access relevant services such as health, welfare, and employability provision
- Maintain accurate and timely case records in line with organisational and regulatory requirements
