Salary: SCP 20–23 (£25,877.80 – £27,925 per annum)

About the role
We are looking for a Health and Wellbeing Co‑ordinator to support adults with complex needs to live well and achieve their personal goals. The role focuses on promoting physical, emotional and social wellbeing through meaningful activities, group sessions and one‑to‑one support.
You will work closely with care and support teams to deliver person‑centred, recovery‑focused outcomes, act as a named keyworker, and use tools such as the Outcome Star to co‑create personalised support plans.

Who are we looking for?
The ideal candidate will:
- Have experience working within health and social care
- Be committed to person‑centred, trauma‑informed practice
- Have experience of team working and/or lone working
- Have experience of co‑producing support plans
- Be motivated to deliver high‑quality, outcome‑focused support
