At West Craigs Supported Living Service, we're looking for an enthusiastic, compassionate, and motivated Health and Wellbeing Coordinator to join our growing team and help transform lives every day.

As a Health and Wellbeing Coordinator, you'll be more than a support worker – you'll be a coach, advocate, leader, and champion for the people we work for. You'll help individuals shape their futures, achieve meaningful personal outcomes, and become active citizens in their communities.

What You'll Do

  • Co-design personalised outcome plans that focus on what matters most to each individual.
  • Support people to live independently, make informed choices, and reach their aspirations.
  • Promote positive mental health, wellbeing, community connection, and inclusion.
  • Build strong relationships with families, carers, health professionals, and partner agencies.
  • Use strengths-based approaches and outcome-focused tools to celebrate progress and success.
  • Provide day-to-day leadership, coordinate tasks, and support colleagues to deliver outstanding care.
