As a Health and Wellbeing Coordinator, you will play a vital role in delivering high-quality support that empowers individuals to achieve positive outcomes, improve their wellbeing, and build confidence in all areas of their lives.

About the role

Working across the Wayfinder Housing First service, you will develop positive, trusting relationships with the people we support, helping them to overcome barriers and achieve greater independence. You will take a holistic approach to wellbeing, working alongside individuals to identify goals and access the support they need to succeed.

No two days are the same. You will support people with a range of practical and emotional needs, working collaboratively with partner agencies and local services to ensure the best possible outcomes.

Key responsibilities

You will:

• Deliver person-centred, trauma-informed support tailored to individual needs.
• Support people to sustain their tenancies and live successfully in their own homes.
• Promote positive physical, emotional and mental wellbeing.
• Assist individuals to access healthcare, welfare benefits, education, training and employment opportunities where appropriate.
• Support people to develop daily living skills, confidence and independence.
• Build effective partnerships with external agencies and community organisations.
• Encourage community participation and social inclusion.
• Maintain accurate records, support plans and risk assessments.
• Safeguard individuals and contribute to the delivery of safe, high-quality services.
• Promote the values and principles of Housing First in all aspects of your work.

About you

We are looking for someone who is passionate about supporting people to achieve positive change and who understands the importance of delivering support with dignity, compassion and respect.

The ideal candidate will have:

• Experience of supporting vulnerable individuals, ideally within homelessness, housing support, social care or a related field.
• A person-centred and strengths-based approach to support.
• An understanding of trauma-informed practice and the challenges faced by people experiencing homelessness.
• Excellent communication and relationship-building skills.
• The ability to motivate and empower individuals to achieve their goals.
• Strong organisational skills and the ability to manage a varied workload.
• The ability to work independently and as part of a team.
• A commitment to equality, inclusion and promoting positive outcomes.
