WAYfinder provides person-centred outreach support to people who are at risk of homelessness, experiencing homelessness, or who have previously experienced homelessness. Our focus is on helping people achieve stable and sustainable housing, improve their wellbeing, and access the right support at the right time.

We take a strengths-based, trauma-informed approach, working alongside individuals to build on their abilities, overcome barriers, and achieve outcomes that matter to them. Through trusted relationships, practical support, and partnership working, we help people navigate challenges and move towards greater independence.

As a Health and Wellbeing Coordinator, you will play a key role in delivering flexible, responsive support within communities, accommodation settings, and local services. You will work collaboratively with individuals and partner agencies to improve wellbeing, promote social inclusion, and prevent homelessness wherever possible.

What you'll be doing

  • Deliver person-centred support tailored to individual needs and goals.
  • Build positive, trusting relationships with the people you support.
  • Help individuals access housing, health, welfare, and community services.
  • Promote wellbeing, resilience, and independence.
  • Support people to engage with local communities and strengthen support networks.
  • Work in partnership with external agencies to coordinate effective support.
  • Complete support planning, risk assessments, and case recording.
  • Contribute to positive outcomes that help individuals maintain stable housing and improve their quality of life.

Who are we looking for?

We are looking for someone who is compassionate, motivated, and committed to helping people achieve positive change.

You will have:

  • Experience within homelessness, housing support, social care, community support, or a related environment.
  • A person-centred and strengths-based approach to supporting others.
  • Strong communication and relationship-building skills.
  • The ability to work confidently with people experiencing complex life challenges.
  • Experience of partnership working and multi-agency collaboration.
  • Good organisational skills and the ability to manage a varied workload.
