We are looking for an experienced, motivated and compassionate Health and Wellbeing Team Leader to join our Stirling & Grangemouth Self-Directed Support (SDS) service and play a key role in delivering high-quality, person-centred support. Our SDS service supports adults with a range of needs, including learning disabilities, physical disabilities, autism and mental health support needs, helping people live independently in their own homes and communities.

As Team Leader, you will inspire and support a team of Health and Wellbeing Assistants and Coordinators, ensuring the people we support receive outstanding care that promotes choice, independence and positive outcomes.

About the role

This is an exciting opportunity for an experienced social care professional who is passionate about leadership and delivering excellent services.

You will:

• Lead, motivate and develop a team of support staff.
• Promote a culture of person-centred, strengths-based support.
• Ensure care and support plans are implemented effectively and reviewed regularly.
• Support individuals to achieve their personal goals, maintain independence and engage with their communities.
• Monitor quality, compliance and service standards in line with Care Inspectorate requirements.
• Manage staffing resources, including rotas, supervision and performance support.
• Build positive relationships with families, social work teams, health professionals and external partners.
• Contribute to continuous service improvement and positive outcomes for the people we support.
• Take part in the on-call rota as required

About you

• Experienced in health and social care, ideally within learning disability, mental health, autism or community support services.
• Confident managing teams and supporting staff development.
• Skilled in problem-solving and decision-making.
• Organised, resilient and able to manage competing priorities.
• Committed to promoting dignity, inclusion, equality and rights-based support.

Essential requirements

• SVQ Level 3 in Health and Social Care (or equivalent).
• Willingness to work towards or hold a relevant leadership qualification.
• Experience of supervising or leading staff within a care setting.
• Knowledge of Self-Directed Support principles and person-centred practice.
• Registration with the Scottish Social Services Council (SSSC), or ability to achieve registration within required timescales.
• Full UK driving licence and access to a vehicle would be advantageous.
