We are looking for an experienced and compassionate Health and Wellbeing Team Leader who can inspire others, champion innovation, and lead the delivery of outstanding person-centred support.

About the Role

Reporting to the Health and Wellbeing Manager, you will have responsibility for the day-to-day leadership of services, ensuring the delivery of high-quality, personalised support that enables people to achieve their goals and aspirations.

You will work closely with the people we support, families, colleagues and external partners to co-create services that improve health, wellbeing, independence and resilience. You will promote a culture of trust, excellence and innovation while supporting your team to deliver positive outcomes every day.

What You'll Be Doing

• Leading and motivating a team of Health and Wellbeing Assistants.
• Promoting a culture of co-creation, participation and person-centred practice.
• Supporting people to achieve the outcomes that matter most to them.
• Ensuring outcome plans are meaningful, individualised and regularly reviewed.
• Coaching, supervising and developing colleagues through outcome-based supervision and workforce development.
• Building strong partnerships with families, communities and external agencies.
• Supporting community integration and helping people maximise local opportunities and resources.
• Driving quality assurance, continuous improvement and service excellence.
• Championing equality, diversity and inclusion in everything you do.
• Supporting recruitment, induction, training and performance management activities.

Essential Requirements

• SVQ Level 3 Health and Social Care (or equivalent) or working towards it.
• Evidence of continued professional development.
• Professional registration with the SSSC.
• Ability to work flexibly to meet the needs of the people we support.
