You will collaborate closely with skilled professionals from health and social care, contributing to assessments, care planning, and interventions that enhance quality of life for individuals and their families.
If you are passionate about improving dementia care, thrive in a collaborative environment, and want to be part of an innovative approach to integrated services, we would love to hear from you.
There is a responsibility for the post holder to demonstrate a commitment to quality service delivery through continuous improvement for the benefit of the service and the organisation.
