Job Description

Internal vacancy – applications for this post will only be accepted from current employees of Aberdeenshire Council (this includes relief staff and agency staff who are currently undertaking work with us).

Within the context of an enablement approach and operating within an integrated service, to provide outcome focussed assessments that inform the provision and co-ordination of enabling packages of support.

To commission and co-ordinate personalised and outcome-led packages of support that promote independence.

To work in partnership with service users, carers, family members, third sector and health partners to provide service users with holistic support.

There is a responsibility for the post holder to demonstrate a commitment to quality service delivery through continuous improvement for the benefit of the Service and the organisation.

The post is based at the Discharge Hub in Aberdeen Royal Infirmary however the post is flexible and will involve some home working.

Fixed term for a period not exceeding 6/02/2026.

Requirements

This post is regulated work with Children and/or Protected Adults under the Protection of Vulnerable Groups (Scotland) Act 2007. The preferred candidate will be required to join the PVG Scheme or undergo a PVG Scheme update check. Where an individual has spent a continuous period of 3 months or more out with the UK in the last 5 years, an Overseas Criminal Record Check will be required. You will be required to provide this check. A confirmed offer of employment and commencement in the post will be subject to the outcome of both these pre-employment checks being deemed satisfactory.

This post has a minimum requirement of 2 references one if which must be your current or most recent employer.
