Job Description
Co-produce a support plan to meet their outcomes identified through an Adult Carer Support Plan or Young Carer Statement.
Support them to make an informed choice on how they would like to direct their identified outcomes via self directed support.
Review the effectiveness of the support via regular review and monitoring process.
There is a responsibility for the post holder to demonstrate a commitment to quality service delivery through continuous improvement for the benefit of the Service and the organisation.
The post holders will be located within the Self Directed Support & Carer Support Team within Aberdeenshire Health and Social Care Partnership but will support both adult and young carers. This post is to cover the North of Aberdeenshire along with another colleague and is part of a team of six Social Worker/ Care Managers covering the whole of Aberdeenshire.
Interviews are expected to take place on 29th and 30th April 2024.
