Job Description

Based at Kinloch Care Centre, Carnoustie covering the South West Angus locality (Carnoustie & Monifieth) working 29 hours per week.

or

Based at Bruce House, Arbroath covering the South East Angus locality (Arbroath and surrounding area) working 36.25 hours per week.

This job is workstyle 3 - Flexi

You will have the opportunity to benefit from our flexible and agile working practices. This means you will work from a variety of locations including working from council buildings and you can work from home to best suit your work, meetings and collaboration.

Working as part of a team, you will undertake needs-led assessments for adults and carers living within the community, identifying and prioritising support needs in line with relevant policies and procedures. You will develop and review support plans, arrange and monitor care packages, and manage individual service user budgets to ensure people receive the right support to maintain their independence.

You will provide information, advice and support to service users and carers, promoting choice, participation and partnership working. The role requires effective liaison with colleagues, care providers and partner agencies to achieve positive outcomes and respond appropriately where care arrangements are at risk of breaking down.
