Job Description

Based at Whitehills Health & Community Care Centre, Station Road, Forfar.

We are currently recruiting for an inspiring social worker to join Angus Health and Social Care Partnership. We are looking for someone who shares in our ambitions and is committed to ensuring that service users receive enhanced standards of care and support.

You must have knowledge and experience of social work services and demonstrate you have the skills and knowledge to be able to make decisions which support our policy framework, standards and service objectives. 

Last year saw the creation of the Angus Hospital Discharge Team which consists of Social Workers / Care Managers and Home Care Assessors. Due to the success of the team, we are now looking to recruit further social workers.

The team covers the whole of Angus, covering Arbroath Infirmary (ARI), Whitehill's Community Care Centre (WHCCC), Royal Victoria and Ninewells Hospitals supporting Angus residents. As part of the team, you will become an integral part of the Multi-Disciplinary Teams (MDT) in these hospitals working very closely with the MDT to aid people to leave hospital safely, and by their Planned Date of Discharge (PDD). 

You will continue to work with people, families, and carers post discharge and ensure safe transfer to a locality teams. This busy team works dynamically to established timescales managing often complex cases. You will ensure robust delivery of practice mainly in relation to statutory and AHSCP policy requirements of Adult Support and Protection, Adults with Incapacity, and Self-Directed Support legislation across the service.

You will work in conjunction with local multi-agency arrangements to promote the achievement of positive outcomes for service users. Actively demonstrate, promote and uphold social work values and deliver fair and evidence-based decision making.
