A full-time permanent vacancy has arisen for a Social Worker in the Angus Discharge Team. The team’s focus is working alongside people who have encountered admission to hospital and require additional care and support in order to enable them to remain as independent as possible when they return to the community. We work with adults and their carers who would benefit from social work support, alongside a variety of agencies and services across Angus.
We are keen to attract enthusiastic, creative and hard-working professionals who can facilitate person-centred planning and outcomes led support plans in a timely, safe and efficient manner, whilst also promoting the rights of the people that we work with. This often includes crisis response to meet the needs of the most vulnerable in society who may require statutory safeguarding measures guided by local policy and procedures.
