Home Accessibility Referral Team (HART)
Salary: £34,452 - £40,501
Hours: 36 per week
An exciting opportunity has arisen to join the Home Accessibility Referral Team (HART) as an Assessment Officer.
The role of HART is to assess housing applicants who have mobility issues, to ensure that they are awarded Gold Priority in order to access social housing that meets their needs. Due to an increase in the number of applicants that are also assessed as homeless, the team is expanding to include an additional Assessment Officer to ensure that applicants with mobility issues can be assessed as quickly as possible and supported to access a home that meets their needs.
To take this work forward, there is a fixed term opportunity to join HART as an Assessment Officer for 12 months.
Tasks and activities of the HART Assessment Officer include:
- Screening EdIndex forms to establish if housing applicants meet the criteria for Gold Priority
- Undertaking housing needs assessments for applicants that meet the criteria
- Liaising with the HART Occupational Therapists around assessed needs and recommendations on property types
- Supporting applicants to access suitable homes
- Nominating applicants for accessible Council homes during the current Allocations Policy suspension
- Providing housing options advice
- Managing a caseload
- Working as part of a closely knit team to develop and improve service delivery
