An exciting opportunity has arisen to work within the Young Adults Disability (YAD) Citywide Social Work Team. This service provides a vital service to Young people transitioning from Children to Adult Services.
YAD are a specialist team of Social Workers who support young people with a severe (diagnosed) learning disability, who may also have complex needs or profound and multiple learning disabilities (PMLD) to:
determine areas of unmet need and personal outcomes
Work alongside Children and Families social workers early within a persons transitions pathway to assess eligible need and plan for a timely transition
link with community resources
identify if a support budget may be required to meet individual needs and outcomes, and make this request
explore all self-direct support options to maximise choice and control
manage the planning and transitional arrangements for anyone who requires care and support
carry out a reviews
YAD is currently a small team of four Social Workers and a Senior Social Worker, however the service is undergoing a restructure and will expand its resource in the near future as well as widening the criteria.

About You
We are looking for a qualified Social Worker who is committed to making a real difference to young people's lives, who are energetic, imaginative, creative, and resilient. Ideally, we are seeking someone with experience of working with people with a Learning Disability and with council officer experience.
Trauma informed, relationships-based and strengths based approach is at the heart of our practice. Adhering to the Social Care (Self-directed Support) (Scotland) Act 2013 to provide a range of choices to people for how they are provided with the support they need.
You will undertake a range of statutory tasks including:
Assessments and support planning
Risk assessment and risk management
Carers' assessments
Duties under the Adult Support and Protection (Scotland) Act 2007 and Adults with Incapacity (Scotland) Act 2000
You will work as part of a multidisciplinary team and will support adults, their families, and carers, to balance the complex areas of risk and need with choice and rights. You will also be able to provide advice, support, guidance and, if necessary, interventions, should an adult be at risk of harm and in need of protection.
You will work in supportive, well-established teams and have regular supervision and training.
Requirements
Social Work roles (SW / SSW): Degree/diploma or equivalent qualification in Social Work. Must be registered (or in the process of registering) with the Scottish Social Services Council (SSSC).
These posts are exempt under the Rehabilitation of Offenders Act 1974 (Exclusions and Exceptions) (Scotland) Order 2013 (as amended). Your PVG fee and SSSC fee will be paid by us (where applicable).
This post is regulated work with children and/or protected adults under the Protection of Vulnerable Groups (Scotland) Act 2007. The preferred candidate will be required to join the PVG Scheme or undergo a PVG Scheme update check. Where an individual has spent a continuous period of 3 months or more out with the UK in the last 5 years, an Overseas Criminal Record Check will be required. You will be required to provide this check. An unconditional offer of employment and commencement in the post will be subject to the outcome of both these pre-employment checks being deemed satisfactory.

Our salary range typically reflects the initial starting salary and annually increases until it reaches the top of the range.
