Social Work Assessment and Support Co-ordinator (16/1443)

A full time and permanent Assessment and Support Co-ordinator post has become available within Adult Social Work in the Complex Care Team.

Your primary responsibility will be to work in partnership with the person and formulate or help facilitate support/care provision. This will be in conjunction with service users’ families where appropriate, and alongside colleagues, other agencies and disciplines in the community and hospital.

You will need to be able to work under your own initiative and prioritise accordingly as well as being an integral member of a busy adult team. You will preferably have a knowledge base that encompasses aspects of social care for adults with priority needs, and their support plan solutions will involve a Personalised Approach in line with departmental policy, good practice and the legislative requirements of the Social Care (Self Directed Support) (Scotland) Act 2013. You will also be expected to practice using protective legislation, in particular the Adult Support and Protection (Scotland) Act in addition to NHS & Community Care legislation.

Experience of working in a health or social services setting is essential and a good working knowledge of the Mosaic system is desirable. Educated to SCQF Level 7 in Health and Social Care or equivalent which evidence of relevant learning in respect of Health and Social is essential and SCQF Level 8 and above in Health and Social Care or equivalent which evidence relevant learning in respect of Health and Social is desirable. Experience of assessment and care management is desirable. You must have excellent communication skills, both verbal and written. Experience of working with Adults with Learning Disabilities or Neurodivergent conditions is desirable.

You will receive regular supervision from the Senior Social Worker and will be part of a very experienced and supportive team. Training opportunities will be available as they arise in addition to ongoing support from team colleagues.

A current driving licence is an essential requirement of this post and you will use Dumfries and Galloway Pool cars if required to work within the community. However, if you have a disability which precludes you from holding a driving licence consideration will be given if alternative transport can be arranged.
