Job Description

With minimal hybrid and based in Dundee and working within agreed GP practices, you will work full time, 37 hours per week, if under Dundee City Council contract, and 37.5 hours per week if under NHS contract. The successful candidate can choose to be employed either by NHS Tayside or Dundee City Council. Requirements

You will have a degree or equivalent in relevant area and documented evidence of post-graduate training.

Responsibilities

You will take patient referrals from GPs and other healthcare professionals within designated practices and efficiently organise and manage cases from initial assessment, to progress through to outcomes, which include improved mental wellbeing, increased access to services, and reduced frequent attendance within primary care. You will provide a person-centred service that is responsive to the needs and interests of clients and support them to identify issues, concerns and barriers to change that affect their health, mental wellbeing and quality of life. Working collaboratively with the client, you will support them through decision making processes to agree the issues they would like to address and activities/services they wish to access. Service development will be part of your role in order to expand the capacity of others to support social prescribing.

The Individual

You will have relevant experience of working with clients on a 1 to 1 basis and in similar situations to the social prescribing service, such as health coaching, caseload management, motivational interviewing, counselling techniques, have comprehensive knowledge of the mental health and Well-being/health inequalities agendas. You will have experience of working with people in a social or health context and of working with people with complex needs. You will be committed to the aims of the Sources of Support (SOS) Scheme, able to make judgements involving complex facts or situations which require analysis, interpretation and comparison of a range of options, to communicate highly sensitive information clearly in writing and orally, have word processing skills/ability to be self-sufficient in terms of administration, meticulous time management skills, able to work in partnership and as part of a team and have analytical and problem solving skills. You will occasional evening working will also be required. This post is considered Regulated Work with Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. The preferred candidates will be required to join the PVG Scheme, or undergo a PVG Scheme update check, prior to a formal offer of employment being made by Dundee City Council.
