Days and Hours of Work:
Working between Monday - Friday, 09:00-17:00

We are looking for a highly motivated individual to work with us in out Adult Social Work Team , East Lothian Health and Social Care Partnership.

We are currently seeking a dedicated and compassionate Community Care Worker to join our dynamic Adult Social Work Team. Our team provides support to adults across a wide spectrum of needs, including:

  • Older adults
  • Individuals with acute and enduring mental health conditions
  • People living with physical disabilities
  • Adults with autism and neurodiverse needs

As a Community Care Worker, you will play a vital role in:

  • Holistic assessment and person-centred care planning
  • Case management and regular reviews
  • Participating in our fast-paced duty system to respond to urgent needs

Job Details

Co-ordinate the provision of a range of services to meet the assessed needs of individuals requiring assistant, to enable them to maintain and maximise their independence.

Job Requirements

Relevant Qualifications
Educated to HNC or SVQ level 3 in social care, or equivalent, and be able to demonstrate equivalent knowledge, skills and competencies gained through relevant experience.

Driving Licence
As the duties of this role require you to effectively travel to successfully undertake the full remit of the role, you must hold a current driving licence and have access to a vehicle.*

PVG Membership
This post is considered as a Regulated Role with vulnerable children and/or protected adults, as specified in the Disclosure (Scotland) Act 2020.
