Job Description

 An opportunity has arisen for the position of Occupational Therapist working within the Assessment Service area of Social Work.
 
Reporting to the Team Leader (Reablement), you will be responsible for the provision of a Reablement Occupational Therapy Service to the citizens of Glasgow.
 
The main duties and responsibilities will include the screening and management of referrals to the Reablement Service. In addition, you will be responsible for the initial Occupational Therapy assessment and subsequent reviews of individuals, including the provision of equipment and minor adaptations. You will be expected to manage a caseload with high demand and throughput. You will work on an agile basis across the city. 
The focus of your intervention will be on functional assessment, goal setting in order to maximise independence for the service users, incorporating falls prevention, physical activity and promoting good continence care. To do this, you will work in partnership with the Assistant Area Operations Manager within Care Services, Home Carers and the local rehabilitation teams to achieve the best outcomes for service users.  
You will be responsible for completing the moving and handling risk assessment in line with current policy and procedures. 
 
To succeed in this role, you must be able to demonstrate working knowledge of relevant legislation, policies and procedures, in line with the Reablement Model in Glasgow. 
The successful applicant will have a flexible approach to service delivery and demonstrate the ability to communicate effectively with internal/external customers and other agencies. You will be required to work on your own initiative as well as part of a team and possession of a UK Driving Licence is desirable.
 
These posts are excepted under Rehabilitation of Offenders Act 1974 (Exclusions and Exceptions) (Scotland) Order 2013 (as amended).
Membership of the Protection of Vulnerable Groups Scheme will be essential on taking up appointment along with HCPC registration. Please note successful candidates for the above position will be required to pay for their PVG Membership/or Scheme Check
 HCPC Registration is also a requirement
