Job Description

Role – Social Care Worker (Complex Needs Services)
Location – GHSCP, 55 Hunter Street, Glasgow, G4 0UP
Contract Status – Permanent

An exciting opportunity has arisen for a Social Care Worker within the Complex Needs Service in Adult Services. This service supports a vulnerable and high-risk population with multiple and complex health and social care needs.

Delivered through a partnership model within the Glasgow City Health and Social Care Partnership, the service uses a Multi-Disciplinary Team (MDT) approach to provide assessment, care, and treatment. Based in Glasgow City Centre, the team engages with service users through an extensive outreach model across various locations in the city.

Key responsibilities include:
• Conducting initial assessments, care planning, and case management for a complex and high-risk caseload.
• Working collaboratively with a team that includes Social Care and Nursing staff, Psychology, Psychiatry, and Occupational Therapy professionals.
• Engaging in multi-agency working
• Carrying out outreach work across Glasgow to promote and sustain engagement with service users.

What we’re looking for:
• Excellent interpersonal and communication skills.
• Ability to work effectively with service users and collaboratively with colleagues and external partners.
• Experience in trauma-informed and person-centred approaches is desirable.

Membership of the Protection of Vulnerable Groups (PVG) Scheme is essential on taking up appointment. Please note successful candidates for the above role will be required to pay for their PVG Membership/or Scheme Checks.

These roles are excepted under Rehabilitation of Offenders Act 1974 (Exclusions and Exceptions) (Scotland) Order 2013 (as amended).
