Salary: Band 5 - £31,982 - £39,735 p.a.
Salary placing will normally be at the first point of the scale.

Job Purpose: Working as part of a Family Team to deliver universal child health programmes for school-aged children and young people (5 - 19 years) within school and community settings including the planning coordination of the school age immunisation programme.
In conjunction with the School Nurse, working with education colleagues in their role as the Named Persons and contributing to the development of the Child’s Plan through health assessment using the national practice model. Working as part of a Family Team to deliver universal child health programmes for school-aged children and young people (5 - 19 years) within school and community settings including the planning coordination of the school age immunisation programme. In conjunction with the School Nurse, working with education colleagues in their role as the Named Persons and contributing to the development of the Child’s Plan through health assessment using the national practice model.
