Job Summary

These posts form part of North Ayrshire HSCP’s ongoing commitment to strengthening and expanding our Discharge to Assess (D2A) Care at Home Reablement Service. The investment in these roles will help consolidate and grow our reablement workforce, enabling more people to return home safely and independently following a hospital stay. By increasing the capacity of our D2A service, we aim to support timely hospital discharges, reduce delays in care transitions, and provide person-centred reablement support that helps individuals regain confidence, skills and independence within their own homes. These roles offer an exciting opportunity to be part of a service that plays a vital role in improving outcomes for people and supporting the wider health and social care system.

Our Workstyles
