The HART D2A Co‑ordinator plays a key role in supporting safe, timely discharge and short‑term assessment pathways for adults returning home from hospital.
Perth & Kinross is split into 3 locality areas (North, South and City) and the post holder will be responsible for coordination, inclusive of organising and coordinating packages of support to vulnerable service users, ensuring timely and effective delivery of services.
This scheduling will be for all service users in their locality and done via Carelink, prioritising referrals from hospitals, also end of life / palliative care, and supporting these in a timely manner, whilst liaising with assessing workers, service users, other professionals both within social work and health regarding packages of care.
Acting as the central link for day‑to‑day coordination, the Co‑ordinator helps maintain positive outcomes for individuals, supports flow across the system, and provides line management, support and guidance to 10 Reablement Assistants.
Flexibility across all locality areas is essential to ensure effective service delivery and coverage.
This is a full time position working Monday – Friday, and 8.45am – 5.00pm. As part of the service requirements, you will be required to undertake duty, approx. 1 per fortnight, participating in daily duty which would mean starting at 7.00am but finishing early that day.
There will also be a requirement to support with weekend cover to meet service demands, however, this would only be approximately 3 weekends per year and this would be done on a rota basis.
This post will be a mixture of Locality Working, Pullar House in Perth and Home Working.
The post‑holder contributes to a responsive, person‑centred service that enables people to remain safely at home while their needs are assessed.

Skills & Experience Required
Strong organisational, planning, and scheduling skills, with the ability to prioritise high‑volume referrals, coordinate complex care arrangements, and manage time‑sensitive tasks.
Line‑management experience, including supporting staff day‑to‑day, offering guidance, delegating tasks, and contributing to supervision‑related responsibilities. The role requires providing daily oversight to Reablement Assistants ensuring consistent practice and safe, high‑quality care delivery.
Excellent communication and relationship‑building skills, working collaboratively with service users, families, hospitals, Locality Integrated Teams, HSCP partners, and the wider HART workforce.
Confident and proactive decision‑making, particularly in fast‑moving situations such as hospital discharge and urgent support planning.
Good digital skills, with the ability to use systems such as Carelink and maintain accurate, high‑quality data to support safe care delivery.
Ability to work independently and as part of a wider locality team, contributing to effective service flow and supporting colleagues across the D2A model.
Values‑driven, person‑centred practice, aligned to reablement and Home First principles, with a focus on enabling individuals to maximise independence and achieve personalised outcomes.
Experience supporting vulnerable adults in community‑based, social care, care management, or reablement settings.
You must have a valid UK driving licence as this post requires you to travel to a variety of locations across Perth and Kinross.
Essential Qualifications
Practice – Essential to hold:
- SVQ Social Services and Healthcare SCQF Level 7 (Was SVQ3).
Supervisory / Management – Essential to hold or be working towards:
- PDA Health and Social Care Supervision (essential or working towards, as a Conditions, and within 3 years of SSSC Registration)
or - A qualification that contains at least 15 credits at SCQF level 7 of supervision or management theory and practice specifically for a supervisor of a care service.
- SSSC Registration within 3 months for: Supervisor in a Care at Home Service

This role requires you to be registered with the Scottish Social Services Council (SSSC), if you are not yet registered, you can still apply, as long as you are willing to apply for registration within 3 months of starting with us and be registered within 6 months.
