We are seeking an experienced, enthusiastic, and motivated Team Leader to lead our Wellbeing Support Team and Social Prescriber Team. As Team Leader you will be joining two established team, developing and shaping the redesign as part of the integrated Primary Care Mental Health Pathway, bringing both teams together as one.

The Wellbeing Support Team delivers short term, recovery focused support for people with low/moderate mental health needs, encouraging the development of self-management skills to support service users to move on from, or live better with, the symptoms of mental ill health. Support is delivered on an individual or small group basis.

Social prescribers offer a person-centred approach empowering individuals to link with appropriate community based, third sector, statutory and local resources to promote health, wellbeing and independence. The aims of the service are to improve quality of life, reduce social isolation and lessen demands on frontline health and social care services. It is an early intervention and prevention service and focusses on enabling clients through an informal approach to encourage resilience and independence. The Social Prescribers work across the whole of Perth & Kinross.

You will provide support and supervision to the team. You will be responsible for all Adult Support & Protection screening and allocation, along with other operational duties such as staff management, work allocation, report writing and data collection. Reporting to Local and Clinical Governance forums, along with annual reports such as the Chief Social Worker’s Report. You will also be expected to attend and participate in locality governance meeting, strategy meetings various partnership forums.

You will be skilled in developing and maintaining effective working relationships with individuals, carers, and other agencies, and be able to sustain positive inter-agency working relationships.

You must be able to demonstrate initiative and independence, to be able to work within defined policy guidelines and be able to deal personally with predictable, and unforeseen, challenges as they arise.

Requirements

You should hold a degree in Social Work, or equivalent, and be a professionally registered practitioner, as per SSSC requirements. A post qualifying qualification in Adult Protection, Mental Health Officer, or Practice Education is desirable, but is not essential. You must have two years’ experience of working at Team Leader level.

Experience of Social Work practice, supervision and line management of staff as a Team Leader is essential. Experience of Adult Support and Protection work is essential.

You should have the ability to effectively manage a busy team, work to timescales and manage competing demands. You will have excellent written, IT and verbal communication skills and demonstrate skills and experience in assessment, and preparation of reports.

You will be highly motivated, innovative and flexible in your approach, as well as being clear and concise in both your written and verbal communication.

You must have a genuine interest in working with people in a recovery focussed manner to improve their mental health and wellbeing.

You will be expected to travel efficiently between various work locations. Due to the rural nature of Perth and Kinross, a full driving licence is therefore required.
