Role Purpose

An exciting opportunity is available to become a part of a newly formed social work team in the Scottish Borders. The Scottish Borders Health and Social Care Partnership have been on a journey to redesign services so that the voices of Young Adults, their families and carers are at the heart of what we do. We have a strengths-based approach, which prioritises walking beside people so that together we can deliver and achieve better outcomes. We have a very exciting opportunity for an experienced, creative and motivated Social Worker/Nurse Care Manager with a passion for achieving excellence to strengthen and support the delivery of a new service.

The post is in the new Young Adults Service with the focus on supporting young adults to reduce risk, reach their potential and transition to adult services.

This is a community based, integrated social care team, providing a single dedicated specialist service for Young Adults (16 – 25 years) with needs such as a Learning Disability, Physical disability, substance/alcohol misuse, family relationship difficulties (history of trauma/abuse), learning difficulties, involvement with Criminal Justice, Mental Health needs (open to CAMHS and transitioning to Adult Community Mental Health Team for ongoing support) and/or subject to Child Protection Procedure. The criteria stated will be impacting on the young person’s daily life and function.

The role involves working across children’s and adult services. You will work creatively and innovatively to assess, plan and coordinate support for young adults to reduce risk, optimise positive outcomes, engage in post school opportunities and to offer support to their families and caring network during this transition.

 

Essential

Social Work Degree or equivalent Health/Social Care Qualification

A current driving licence and use of a vehicle is an essential requirement of this post.
