Successful candidates will work in conjunction with people who use services who have a disability. Partners that you will make connections with include family carers, day service colleagues, assessment and care management staff, and other agencies in order to support people to maximise and maintain individual potential.
Successful candidates will promote social inclusion of all people who use services within the community as well as working within our resource buildings.
Successful candidates should hold a SVQ Level 3 in Social Care or a HNC in Social Care, and if you don’t already have a relevant qualification, you should be willing to work towards this.
Requirements
Duties of the post include:
- Working as part of a team in the development of outcomes focussed activities for groups and individuals
- To contribute to assessment and care management process by providing information to the case managers to inform individuals’ co-produced assessments
- Undertake a key worker role by implementing, monitoring and reviewing individual care and support plans
- Co-ordinate and prepare review meetings and complete review minutes.
- To work in conjunction with service users to assess their skills, abilities and needs with a view to identifying opportunities, for personal and social development and to provide ongoing support where necessary
- To liaise and work in partnership with other Council Resources, external agencies, family carers and community groups to access appropriate supports and opportunities for people who use services
- To plan, develop, facilitate and record both group and individual activities
- Maintain records to a professional standard and in line with Council guidance in accordance with case recording guidance
- To undertake Risk Assessments under the guidance of a line manager
- To work with individuals’ personal carers in the implementation of daily activities
- To assist and/or take responsibility for administering medication to people who use services

Foreign Police Check
Candidates should be able to detail any gaps in employment and provide a foreign police check where applicable for any periods living or working abroad.
