An opportunity has arisen for a highly motivated individual to enhance their skills relating to adults with incapacity case work. Duties will include: co-ordinating active AWI cases, ensuring timely communication with families, carers and private law firms, in line with AWI procedures and best practice in order to minimise any delays or barriers to discharge. You will also provide support to the Hospital Discharge Mental Health Officer. 

You will be responsible for updating and maintaining records over multiple platforms i.e. Liquid Logic and Microsoft Excel. 

Your caseload will include a variety of individuals across all acute sites, however, you will be aligned to and supported by the Operations Manager responsible for overseeing NHSGGC hospital social work activity. 

You will be working with Adults aged 16 years and older who will have a range of health and social needs and play a vital role in supporting the MHO and Operations Manager. 

You will be responsible for the completion of a holistic assessment of risks and needs, developing care support plans to meet identified needs, reviewing existing plans and supporting duty work under the supervision of the Operations Manager. This work is underpinned by the principles of the Social Work (Scotland) Act 1968. You will also partake in a duty rota.

The successful candidate will be motivated, organised, willing to learn and have experience of working with vulnerable groups in the community.
