An exciting opportunity has arisen for a highly motivated and dedicated Social Worker to join our Adult and Older People Social Work Team within South Lanarkshire Health and Social Care Partnership. This is a rewarding role where you will make a real difference to the lives of adults and older people across our communities.

You will work with adults aged 16 and over who have a range of health and social care needs. Your role will involve delivering a person-centred, outcome-focused service that promotes independence, choice, and wellbeing.

As a Social Worker, you will take a strength based approach to undertake holistic assessments of risk and need, ensuring that every aspect of an individual’s circumstances is considered. You will develop and regularly review personalised support plans that reflect identified outcomes and promote independence. The role also involves supporting duty work and responding to urgent situations under the supervision of the Operations Manager. All practice must align with the principles of Self-Directed Support (SDS) and comply with relevant legislation. You will work collaboratively with multi-disciplinary colleagues, partner agencies, and voluntary organisations to deliver high-quality, integrated support.
